THE CHALLENGE

 

Discharge from hospital can be planned or advised at short notice ahead of a weekend. Hospitals are keen to fast track discharge for various reasons. Returning to your home without additional support is less than ideal at a time when you want to recuperate and regain your independence.

 

 

 

 

 

HOW CAN WE ASSIST?

 

Meet you at the hospital ahead of discharge and take instructions for preparing your residence, shopping for food supplies and running errands
Accompany you from hospital on your way back home
Assist you in the early hours / days post discharge by extending trusted companionship, provide personal care, liaise with family / Doctor / Pharmacy as well as community nurses, physiotherapists and other allied health professionals.
We are able to put in place nursing oversight either for set time/date or on as on call.
Typically, a single Companion will support you to regain independence within framework of a restorative care model.
Your companion will be only too happy to function as an extended member of your household or a trusted friend. Your companion will do as much or as little as you require. Companions can accompany you to outpatient appointments, take you around to see friends or run errands on your behalf.

Your service delivery manager will coordinate with you a time to meet either pre or post discharge and complete a short term companionship / personal care / household management care plans
Your Companion is assigned and visitation commences with a focus on a restorative model aimed at assisting you in regaining your independence.

Post Hospitalisation Support

Perfect for people:

  • Who have just been discharged from hospital or rehabilitation.
  • Who wouldn’t ordinarily need a Carer but need one at the moment.
  • Who need to be picked up from hospital and need help rearranging the house.

 

6 hours: Arrival 8am - 9am or 30 minutes prior to discharge if required.
  • Meet at hospital / accompany home
  • Assistance with personal care upon arrival, including showering & personal grooming
  • Supervise discharge medication / liaise with pharmacy & GP
  • Prepare and service next meal
  • Change bed linen
  • Report to family / next of kin
  • Assist to rest time
  • Freshen up & light house cleaning
  • Laundry
  • Run errands including, shopping & scripts
  • Document notes & handover
  • Report to family / next of kin
12 hours: Arrival 7.30pm - 8.30pm
  • End of day tidy up
  • Prepare and serve dinner
  • Assistance with evening personal care
  • Supervise evening/night medication
  • Document stay notes & handover

  • Assistance with morning personal care, including showering & personal grooming
